Neurocrine Biosciences shares rose 13.4% on Tuesday, closing at $164.85 from a previous close of $145.35, after the company's third-quarter results beat Wall Street's revenue and profit estimates and management raised full-year sales guidance for INGREZZA, its lead drug.

The reaction was unusually large for this company. Tuesday's rise was 6.5x the size of Neurocrine's typical daily move over the past year, which has run at 2.1%, on trading volume 3.0x the 30-day average. Shares have not risen this much in a single session since 2 November 2017. The move dwarfed the broader market: the S&P 500 added 0.3% that day, and the median move among other covered biotechnology shares was 1.5%.

The quarter behind the rally

Total revenue for the third quarter came to $1.1 billion, up 41.2% on a year earlier and 17.1% on the second quarter, Neurocrine said in its earnings release. That beat the average of analysts' estimates by 7.3%. INGREZZA, used for tardive dyskinesia and Huntington's chorea, brought in $801 million of net product sales, up 17% year-over-year, according to the release. CRENESSITY, for classic congenital adrenal hyperplasia, added $211 million, up 115% year-over-year. Neurocrine also raised its full-year INGREZZA sales guidance to a range of $2.9 billion to $2.925 billion, up from $2.825 billion to $2.875 billion previously.

Profitability moved with revenue on an adjusted and operating basis. Operating income rose 38.6% year-on-year to $331.3 million, operating cash flow rose 73.7% to $395.2 million, and free cash flow for the quarter came to $383.6 million. Non-GAAP diluted earnings per share, the measure Neurocrine highlights, reached 3.29, beating the consensus estimate of 2.75 by 19.8%, the third such beat in the last four quarters. Of the eight quarters before this one, only 1 grew revenue faster.

GAAP profit lagged the headline beat

Reported, GAAP net income was $206.5 million, down 1.4% from $209.5 million a year earlier, and GAAP diluted earnings per share fell 3.9% to 1.96 from 2.04. Net margin fell 8.0 percentage points to 18.4%. Neurocrine's release attributes the gap between GAAP and adjusted profit to a $52 million loss on equity investments this quarter, against a $31 million gain a year earlier, plus acquisition-related and integration costs tied to the Soleno deal that the company excludes from its adjusted figures. The rally therefore rewarded the adjusted growth and guidance news more than the reported GAAP profit, which declined.

What the quarter means for our value model

Neurocrine's revenue acceleration and margin expansion feed directly into the assumptions behind Fundamentalytics' discounted cash flow model. The model's base case assumes growth of 20.0% for its first forecast stage. Holding everything else equal, growth five percentage points lower would move the model's value by −23.4%, while five points higher would move it by 29.9%. A one percentage point rise in the discount rate would move the model's value by −15.8%. Free cash flow margin moves the model by less per point: a one-point fall in margin would change its value by −3.5%, while if the past year's full margin improvement persists, the model's value would move by 26.5%. Based on the quarter's fundamentals, Fundamentalytics' base-case model currently values Neurocrine shares at $280.00. These are model values under stated assumptions, not a comparison with the share price.

What would confirm or undo this

Neurocrine is scheduled to host an R&D day in New York on December 15, 2026, where it is expected to update investors on the pipeline behind this quarter's growth, including Phase 3 readouts flagged in the release for 2027 in osavampator for major depressive disorder and direclidine for schizophrenia. The next quarterly report will show whether CRENESSITY and VYKAT XR keep growing at this pace, and whether the gap between GAAP and adjusted profit narrows as Soleno integration costs roll off.
