Adaptive Biotechnologies raised its minimal residual disease testing outlook after another quarter of expanding clonoSEQ use, despite a decline in total revenue. The apparent contradiction comes from the comparison: the previous year included Genentech collaboration revenue, while the latest quarter included none. Cancer testing grew, and the company generated positive quarterly free cash flow, although it remained just below GAAP operating break-even.
Revenue of $83.9 million fell 10.7% against the same quarter a year earlier but rose 17.3% from the preceding quarter. Neither comparison alone captures the change in the business. Management reported revenue growth of 39% excluding Genentech, showing why the headline decline gives an incomplete account of demand for Adaptive’s continuing activities.
Wall Street had set a lower bar. Analysts had modelled quarterly revenue near $71.6 million, so the reported figure came in 17.2% above that consensus estimate. On earnings, the consensus estimate had pointed to a loss of roughly $0.10 a share; Adaptive instead reported a $0.02 profit, a swing equivalent to a 120.3% surprise.
Adaptive revenue reflects growing clonoSEQ use
MRD revenue reached $72.6 million, up 28% against the same quarter a year earlier. Management reported growth of 44% excluding milestones, which separates the expansion in its core testing activities from payments dependent on particular contractual achievements.
Adaptive delivered 39,587 clonoSEQ tests, an increase of 46%. That supports management’s account of expanding use, although test volume and segment revenue are different measures: the release does not provide a bridge separating reimbursement, customer mix and milestone effects. The figures therefore establish greater testing activity without establishing what happened to revenue per test.
Immune Medicine moved in the opposite direction on a reported basis. Revenue of $11.3 million fell 70%, but management reported growth of 229% excluding Genentech. Adaptive also recognised revenue following successful data delivery under a target discovery agreement. That contribution matters when judging the repeatability of the quarter: successful delivery under a collaboration is a different revenue source from ongoing clinical testing.
The company said work towards separating Immune Medicine was progressing, without announcing completion or providing revenue guidance for that business. The expanding MRD operation is consequently easier to assess against management’s outlook than the future contribution from Immune Medicine.
| Measure | FY2026 Q3 | Comparison |
|---|---|---|
| Revenue | $83.9 million | Fell 10.7% year on year |
| GAAP operating margin | −0.5% | Fell 11.5 percentage points year on year |
| GAAP diluted EPS | $0.02 | $0.06 a year earlier |
| Free cash flow | $7.7 million | −$7.5 million a year earlier |
Profitability depends on the measure used
Chief executive Chad Robins described company-wide profitability as arriving ahead of expectations. Adaptive reported non-GAAP adjusted EBITDA of $14.3 million, but its GAAP operating margin was −0.5%. Positive earnings therefore did not mean that revenue had fully covered operating expenses.
The release puts operating expenses at $84.4 million, compared with $83.7 million a year earlier. With costs broadly stable and reported revenue lower, the deterioration in GAAP operating margin follows directly. Interest and other income, net, of $3.8 million helped produce positive GAAP earnings, while interest expense was $0.5 million.
Non-GAAP adjusted EBITDA also excludes share-based compensation and depreciation, among other items. Adaptive explicitly expects meaningful share-based compensation to continue, so those exclusions should not be treated as costs that have disappeared.
There is nevertheless evidence of improvement within MRD itself. Its segment reconciliation reports positive net income, compared with a loss a year earlier, alongside higher non-GAAP adjusted EBITDA. The testing operation’s progress is therefore visible beyond revenue growth, even though the consolidated comparison remains affected by Genentech.
Cash provides a separate test. Quarterly operating cash flow was $9.6 million, leaving $7.7 million of free cash flow after capital spending, compared with −$7.5 million a year earlier. However, trailing-twelve-month free cash flow remained −$8.8 million, with a margin of −3.0%. The latest quarter establishes a positive cash contribution; subsequent quarters must establish its persistence. The results announcement does not explain the working-capital contribution in enough detail to attribute the improvement entirely to operating profitability.
Higher MRD guidance still depends on reimbursement
Management raised its full-year MRD revenue range to $276.0 million–$286.0 million from $268.0 million–$278.0 million. It also moved expected company-wide operating expenses, including cost of revenue, to approximately $350.0 million from a range of $350.0 million–$355.0 million. Higher expected testing revenue alongside a tighter spending outlook supports the prospect of better operating profitability, although neither constitutes guidance for consolidated net income or free cash flow.
Adaptive highlighted updated multiple myeloma guidelines that support annual MRD testing during maintenance. Separately, the company has asked MolDX to create a new Medicare reimbursement bundle covering that ongoing maintenance testing, a request still pending before the regional contractor. A pending request is not approval, and clinical guideline support does not itself establish payment coverage.
That distinction bears directly on the annual filing’s warning that repeat clonoSEQ use depends on reimbursement as well as physician adoption. Payors can deny claims or limit coverage, potentially delaying collections or reducing demand. The latest quarterly filing reported no material changes to previously disclosed risk factors; the standing reimbursement risk remains relevant as testing volumes expand.
Sustained testing cash flow matters for intrinsic value
The quarter strengthens the case for higher recurring MRD revenue, but the input most directly relevant to our model’s intrinsic value is the cash margin that revenue can sustain. Positive quarterly free cash flow supports that assessment; the negative trailing-twelve-month result prevents treating cash generation as established. The full Adaptive Biotechnologies analysis provides the wider financial history.
No quantified sensitivity is available here to translate this quarter’s improvement into a defensible change in our model value. These are model values under stated assumptions, not a comparison with the share price.
Further results need to show whether expanding clonoSEQ use produces repeated positive cash flow without depending on intermittent collaboration deliveries. For maintenance testing specifically, the unresolved development is whether the MolDX submission secures additional reimbursement coverage.
